INDIVIDUAL MEDICAL ASSISTANCE PROGRAM (IMAP)

Under this PCSO program, medical assistance is given to individual patients through the issuance of guarantee letters to hospitals where the patients are confined. A guarantee letter is a certification issued to hospitals for a particular charity patient under the PCSO medical assistance program, where the agency assumes the obligation of settling the cost of hospitalization, including the medicines, medical, surgical, or blood supplies, and diagnostic procedures.

OBJECTIVES

General : Restoration of social functioning (Physical recovery) through medical assistance
Specific : Provide assistance for hospitalization expenses, laboratory procedures, and purchase of medicines, chemo drugs, dialysis solutions, antibiotics, implants, devices, and other medical needs.

Request your doctor to assist you with the Medical Abstract, Treatment Protocol, Medicine Prescription, and Radiotherapy Needs. Always have a copy of your histopathology results – keep one file for your files.

Cueing at the PCSO requires early arrival (5 am) and much patience.

PCS also refers patients in need to PCSO for assistance.

PCSO HELP DESK IN AFFILIATED HOSPITALS

Specifically known as ASAP (At Source Ang Processing). This program aims to facilitate accessibility in the filing and processing of requests for Individual Medical Assistance Program (IMAP) for indigent patients confined in the affiliated hospital. Patients NEED NOT TO GO TO PCSO office to file their request for assistance. They can immediately file their request and submit the necessary requirements at the PCSO Help Desk.

GMA Kapuso Foundation offers assistance to patients aged 0-18 years old from ANY GOVERNMENT HOSPITALS. The patient must prepare the following documents:

ANDRES SORIANO FOUNDATION
Andres Soriano Foundation offers assistance to patients with BREAST CANCER STAGE I-II or better. Patients must prepare the following:

  1. Profile of patient, diagnosis (original or certified true copy of medical abstract and histopathology or biopsy results), history, plan of treatment/doctor’s report with his name, signature, and license number.
  2. Names of medication needed (prescription of all drugs signed by the medical officer with his/her license number) and prices (include source of price information).
  3. Letter of request for financial assistance from the patient.
  4. Photocopy of the identification card with the latest picture of the requesting person with his/her signature on the back.
