PCS WEBSITE USER POLICY AGREEMENT
This site www.philcancer.org.ph with its home page in the domain "philcancer.org.ph" is the official website of the Philippine Cancer Society, Inc. (PCS) and is thus available to all users of the World Wide Web. Visitors to the website agree to be bound by the terms and conditions set forth in this User Agreement. PCS reserves the right to change these terms and conditions at any time, without notice to you, and you must consult the most recent version of this User Agreement (not an older cached version) each time you view the Website. Use of this Website constitutes your acceptance of all of the following terms and conditions, in their current form or as modified.
The Website service currently provides users access to an extensive amount of information, including, but not limited to, public forums, health and medical content, online tools, calendaring, email, to-do lists, bookmarks, other personalization features, book/publication offerings, and various communication tools. PCS assumes no responsibility or liability for the timeliness, deletion or failure to store any user communications or personalization data or settings.
As an option, users may register and create a personal account profile to provide more personalized information. Users are responsible for maintaining the confidentiality of the user ID and password, and are fully responsible for all activities that occur under their account. ACS does not retain liability or responsibility for such use.
Users acknowledge that PCS may establish general practices and limits concerning use of the PCS web site, including without limitation the maximum number of days that email messages, message board postings or other uploaded content is retained by the web site. The maximum number of emails that can be sent or received by an account, the maximum disk space allotted on servers for an account, and the maximum duration for which an account can be accessed in a given period of time. You acknowledge that PCS has no responsibility or liability for the deletion or failure to store any messages and other communications or other content maintained or transmitted by the PCS web site servers. You acknowledge PCS has the right to log off accounts that are inactive for an extended period of time. You further acknowledge that PCS reserves the right to change these general practices at any time with or without notice.
Registered User Conduct
You understand that all information, data, text, photographs, graphics, messages, or other materials ("Content") publicly posted is the sole responsibility of the person from which such content originated. This means that you, and not PCS, bear all risks and liabilities for all Content that you upload, post, email, transmit or otherwise make available via the service. You agree to not use the service to:
a. upload, post, email, transmit or otherwise make available any Content that infringes any patent, trademark, trade secret, copyright or other proprietary rights ("Rights") of any party;
b. upload, post, email, transmit or otherwise make available any unsolicited or unauthorized advertising, promotional materials, "junk mail," "spam," "chain letters," "pyramid schemes," or any other form of solicitation. (Posting the same note more than once can be considered "spam" or "spamming");
c. upload, post, email, transmit or otherwise make available any material that contains software viruses or any other computer code, files or programs designed to interrupt, destroy or limit the functionality of any computer software or hardware or telecommunications equipment;
d. upload, post, email, transmit or otherwise make available any Content that is unlawful, harmful, threatening, abusive, harassing, defamatory, vulgar, obscene, libelous, invasive of another's privacy, hateful, or racially, ethnically or otherwise objectionable;
e. impersonate any person or entity, including, but not limited to, a PCS official, forum leader, guide or host, or falsely state or otherwise misrepresent your affiliation with a person or entity;
f. forge headers or otherwise manipulate identifiers in order to disguise the origin of any Content transmitted through the website service;
g. disrupt the normal flow of dialogue, cause a screen to "scroll" faster than other users of the service are able to type, or otherwise act in a manner that negatively affects other users' ability to engage in real time exchanges;
h. intentionally or unintentionally violate any applicable local, state, national or international law;
i. "stalk" or otherwise harass another; or
j. harvest or otherwise collect or store personal data about other users.
You acknowledge that PCS and its designees shall have the right (but not the obligation) in their sole discretion to refuse, move, edit, or delete any content that is available via the website. PCS and its designees shall have the right to remove any Content regardless of whether such communication(s) violate the User Agreement. You agree that you must evaluate, and bear all risks associated with, the use of any content, including any reliance on the accuracy, completeness, or usefulness of such content.
You acknowledge and agree that PCS may preserve Content and may also disclose Content if required to do so by law or in the good faith belief that such preservation or disclosure is reasonably necessary to: (a) comply with legal process; (b) enforce the User Agreement; (c) respond to claims that any Content violates the rights of third-parties; or (d) protect the rights, property, or personal safety of PCS, its users and the public.
User Submissions
By submitting communications or content to any part of this Site where such content would be viewable by the public (e.g. posting on a public forum/bulletin board or public chat or other public communication), you agree that such submission is non-confidential for all purposes. Any submission to this Web site will be deemed and remain the property of PCS. You grant or warrant that the owner of such content has expressly granted PCS a royalty-free, perpetual, irrevocable, world-wide non exclusive license to use, reproduce, create derivative works from, modify, publish, edit, translate, distribute, perform and display the communication or content in any media or medium, or any form, format, or forum now known or hereafter developed.
Termination
You agree that PCS, in its sole discretion, may terminate your password, account (or any part thereof) or use of this Service, and remove and discard any content within the Service, for any reason, including, without limitation, for lack of use or if ACS believes that you have violated or acted inconsistently with the letter or spirit of this user agreement. You agree that any termination of your access may be effected without prior notice, and acknowledge and agree that PCS may immediately deactivate or delete your account and all related information and files in your account and/or bar further access to such files or the service. Further, you agree that PCS shall not be liable to you or any third-party for any termination of your access to the website services.

The Philippine Cancer Society (“PCS”) respects the privacy of every individual who visits www.philcancer.org.ph (“Web site”). This notice explains our online information practices. Registration: If you choose to customize the Web site to your needs by becoming a registered www.philcancer.org.ph Web user, we retain the preferences you select so that you will not have to reenter the information each time you access our Web site. These preferences may include requests for email, news, information on specific cancer types, language preferences, and interest group selections (patient, medical professional, volunteer). You can access your profile by entering your user name and password each time you use the Web site. You are not required, however, to enter your name or mailing address.
If you register to participate in a PCS event, such as Relay For Life or Best Dressed Women of the Philippines, using our event registration site, we may ask you if you are a cancer survivor or donor. This information is useful for event purposes. Further, if you elect to use our referral service to inform a friend about the event registration site, we will ask you for your friend’s name and address. We will store this information in order to send your friend a one-time email inviting him or her to visit the site.
Contributions: If you choose to donate to our organization, we maintain a record of your contribution. Your financial information will be treated as “confidential.”
Requests: If you submit an online request for PCS collaterals such as free brochures, or submit a question through a “contact us” query, we may have to gather additional information from you to respond to your request. Such information will vary with the request but often includes name, shipping address, telephone number, and email address.
Public Forums: If you choose to participate in a chat room, discussion board, news group or another public forum that we make available to our users, please remember that any information that you disclose in these areas becomes public information. You should exercise caution when deciding whether or not to disclose your personal information.
Letter to Congress: If you choose to generate an email to your Congressional representative on our site, we receive a blind copy of the email. We may use your letter or information from your letter to further educate Members of Congress about our cancer advocacy priorities.

We limit the use of information provided to us on our Web site to the following:
Internal Use
1. If you do not make a service request, donation, purchase, or otherwise identify yourself, we will have no personally identifiable information about you. We will only use aggregate information derived, in part, from your use of our site to improve our site and our service to you.
2. If you provide personal information, we may enter your name into our constituent database and contact you in order to:
o Complete voluntary surveys seeking feedback for quality and service improvement purposes.
o Supply you with information including cancer related health news, PCS programs, events and services.
o Request voluntary time or monetary contributions to PCS.
o Request your participation in an PCS research or poll study.
3. We collect the email addresses of those who communicate with us by email. Inquiries may be forwarded to the appropriate PCS department for response and may be entered into our constituent database. If your name is entered into the database, we may contact you (see (2) above).
External Use
Your health-related information is privileged and confidential and will not be shared or released to any organization or business entity other than those affiliated with or working in conjunction with PCS as follows:
1. We occasionally make our constituent names and postal addresses available to other reputable non-profit organizations. We have found this to be the most cost-effective method of increasing our database of potential constituents and hope that you value the information they send you. Your name is only available to these carefully screened organizations for a limited time and it is de-identified, such that it is not associated with the Philippine Cancer Society. Other organizations will not have continued access to your name and address unless you choose to respond to their initial mailing. We do not share email addresses or health related data.
2. We occasionally hire other companies to provide limited services on our behalf. We will only provide those companies the information they need to deliver the service and prohibit them from using that information for any other purpose.
3. We have relationships with companies that conduct charitable sales promotions and commercial co-ventures that support us in our mission and activities. If you provide us with your mailing address, we may pass your contact information to these companies so that they may ask you if you are interested in receiving their services. Your choice to use their services will benefit us; the amount of money we receive from these entities as a result of your participation is disclosed at the time you are contacted about the service. You are under no obligation to respond and the companies are restricted from using your contact information for any other purpose.
Your Options
We respect your privacy and allow you to restrict internal and external sharing of your personal information. We recognize that you may wish to limit the ways in which we contact you and we offer the following options:
1. Do not contact me by telephone.
